About Us
We opened our doors in 1992 with the purpose of finding better ways to serve
the community. With over 120 years of combined professional experience, our
medical and chiropractic physicians are here to help you recover from your
injury. As an outpatient treatment center, we are licensed by the Arizona
Department of Health Services (ADHS) and approved by the US Citizenship and
Immigration Services (USCIS) to perform civil surgeon's exams. We
specialize in personal injury care (lien basis) and workers' compensation
cases including supportive care and I-693 immigration medical examination.

- Medical and chiropractic treatments
- Independent medical examinations (IMEs) (e.g. Social Security, workers' compensation, immigration)
- Physiotherapy, ECG/EKG, and X-rays administered on premises
- X-ray examinations and reports read by board-certified radiologist
- MRI, CT, neurology, and orthopedic referrals (lien basis)
- Medical examination required for Form I-693 performed by our civil surgeon
Legal
- Assistance with paperwork for your lawyer, insurance company, workers' compensation case, and Form I-693
- Obtaining of treatment records from other providers for your legal case
- Referral to a reputable attorney to ensure you get compensated for the full amount you are entitled to
